Physical Therapist – Skilled Nursing Facility
Position Overview
Join the rehabilitation team at a skilled nursing facility dedicated to comprehensive post-acute care. As a Physical Therapist, you'll assess and treat patients recovering from surgery, illness, or injury, helping them regain strength, mobility, and independence.
Key Responsibilities
- Conduct thorough physical evaluations and develop individualized treatment plans
- Implement therapy interventions focused on strength, mobility, and functional recovery
- Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
- Collaborate with occupational therapists, nurses, and physicians to coordinate comprehensive care
- Monitor progress and adjust treatment plans to optimize outcomes
- Maintain accurate, timely documentation in compliance with facility policies and regulatory standards
Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license
- Experience in a skilled nursing facility or similar post-acute setting preferred
- Proficiency with physical therapy techniques, therapeutic modalities, and adaptive equipment
- Strong communication and problem-solving skills
- Ability to work independently and collaboratively
