Physical Therapist – Home Health
Position Overview
Join a home health team providing skilled physical therapy services to patients in their homes. You'll manage a dynamic patient caseload, develop individualized care plans, and supervise support staff while maintaining high documentation standards.
Schedule & Duration
- Monday–Friday, 8:00 AM–4:00 PM
- 2–3 weekend days required
- 13-week contract
- 37 guaranteed hours per week
Key Responsibilities
- Conduct comprehensive OASIS and PT assessments, evaluating functional status, safety risks, and psychosocial factors
- Develop and implement individualized plans of care focused on functional independence and safety
- Provide skilled physical therapy treatment, including therapeutic techniques and patient/caregiver education
- Manage patient visit schedules proactively to optimize caseload and patient needs
- Supervise and direct PTAs, PT students, and home care aides
- Coordinate care with physicians, nurses, caregivers, and other rehabilitation services
- Complete all required documentation, including OASIS time points, in compliance with federal and state regulations
- Perform wound care, staple/suture removal, and vital sign monitoring as needed
- Ensure accurate billing documentation and insurance compliance
- Participate in peer review audits and quality improvement initiatives
- Participate in community outreach and training programs
Requirements
- Current PT license (Vermont)
- APTA membership and commitment to professional standards
- Ability to assess patients' needs for home care aide services
- Strong organizational and communication skills
- Experience with OASIS documentation preferred
