Position Overview

Join the rehabilitation team at the facility in Ukiah, CA. You'll plan and conduct treatment programs designed to restore and improve patients' physical, social, and mental functions while serving as a co-leader in an interdisciplinary approach to patient care.

Shift & Schedule

• 8:00 AM – 4:30 PM
• 5 days per week (no weekends)
• 13-week contract
• 40 guaranteed hours per week

Key Responsibilities

• Complete patient evaluations and establish individualized treatment plans using evidence-based clinical techniques
• Provide therapy interventions using appropriate treatment protocols
• Develop and communicate care and discharge plans with the interdisciplinary team
• Maintain comprehensive, timely clinical documentation
• Supervise assigned physical therapy assistants
• Perform additional job-related duties as assigned

Required Qualifications

• Doctor of Physical Therapy (DPT) degree
• Current Physical Therapy licensure in California
• Current BLS/CPR certification
• At least 2 years of Physical Therapy experience

Preferred Qualifications

• 3–5 years of recent PT experience
• Pelvic floor therapy experience
• PRPC or WCS certification (or working toward it)
• Electronic health record system experience
