RN Care Manager – 13-Week Contract

Position Overview

The facility seeks an experienced RN Care Manager to coordinate utilization management activities and serve as the primary link between patients, healthcare teams, and payors during hospital stays. You'll work on a dedicated inpatient unit, managing concurrent case management and discharge planning across the patient population.

Shift & Schedule

• 8:30 am – 4:30 pm
• 40 hours per week
• No call required
• Weekend and holiday rotation required
• 13-week contract

Key Responsibilities

• Assess patient needs and identify resources required for transitions to the next care setting
• Collaborate with interdisciplinary teams during daily care rounds, conferences, and care planning
• Communicate effectively through written, verbal, and electronic systems
• Partner with community organizations to secure durable medical equipment, arrange transportation, and obtain health insurance authorizations
• Participate in length-of-stay management for the inpatient population
• Apply knowledge of hospital reimbursement methods and quality measures relevant to care management

Required Qualifications

• Associate or Bachelor degree-prepared Registered Nurse
• 3–5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review
• Utilization review and discharge planning experience required
• Active BLS certification required
• CCM or ACM certification preferred
• Nationally recognized case management or utilization review certification preferred
