About the Role
You'll deliver comprehensive physical therapy services to patients in a long-term care environment. Your focus will be on restoring mobility, reducing pain, and helping patients maintain functional independence in their daily lives.
Key Responsibilities
- Perform thorough patient evaluations, including history, systems review, and relevant tests and measures
- Develop individualized, patient-centered treatment plans aligned with physician orders
- Conduct ongoing reassessments and adjust care based on patient response
- Manage discharge planning from the initial visit through transition
- Supervise and delegate to ancillary staff and students in compliance with state regulations
- Document clearly and accurately per departmental and regulatory standards
- Communicate and educate patients, families, and caregivers
- Participate in quality improvement and professional development activities
- Maintain a safe environment for patients, visitors, and staff
Requirements
- Texas PT license or compact license (required)
- Minimum 2 years of experience in a long-term care setting (required)
- Current knowledge of state practice act and scope of practice
