Care Coordination RN
Position Overview
The facility seeks an experienced Care Coordination RN to join the team on a 91-week contract. This role focuses on care coordination activities within an acute care setting.
Shift Details
- 8-hour days: 8:30 AM – 5:00 PM
- 2 weekends per month
- 40 guaranteed hours per week
- Orientation dates: October 19 and November 2
Requirements
- Active California RN License
- BLS Certification
- LA City Fire & Safety Certification
- Prior experience with CA Dignity/ health system required — candidates without this background will not be considered
- Demonstrated experience in care coordination
Assignment Details
- Acute care setting
- 91-week contract
- Hospital retains the right to cancel up to 4 shifts per 13-week assignment period (3 shifts for assignments shorter than 13 weeks)
