Clinical Pharmacist – Primary Care

Position Overview

Join a primary care team as a Clinical Pharmacist providing direct patient care and clinical leadership in medication management. You'll work across two primary care clinics, seeing approximately 16 patients per day in a collaborative, multidisciplinary environment.

Schedule & Duration

• 4x10-hour shifts: 07:00–17:00 or 09:00–19:00
• Weekdays only
• 13-week contract
• Start date: October 27, 2026

Required Qualifications

• BCACP board certification and PGY2 Ambulatory Care residency (required)
• Texas State Board of Pharmacy license
• 4+ years of direct patient care demonstrating medication management expertise
• BLS certification
• Flu vaccine required (medical/religious exemptions accepted)

Key Responsibilities

• Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements
• Conduct drug utilization reviews and monitor for drug interactions and adverse events
• Create patient medication action plans with self-management goals; minimize polypharmacy
• Serve as preceptor for pharmacy students and residents
• Provide drug information, dosing, and monitoring services to the care team
• Develop educational materials and in-services for medical, nursing, and pharmacy staff
• Participate in P&T policy development and Joint Commission compliance initiatives
• Communicate clinical interventions as a core member of the patient care team

Work Environment

Onsite primary care setting with extended daytime hours. Time split between two clinic locations.
