Position Overview
The facility is seeking a Case Manager for a 13-week assignment. You'll work day shift (7:00 AM – 3:00 PM) in a collaborative healthcare environment focused on delivering quality patient outcomes and coordinated care.
About the Role
As a Case Manager, you will:
- Work collaboratively across the clinical team to support patient transitions and care planning
- Utilize strong documentation skills to maintain accurate, timely records
- Communicate clearly and compassionately with patients, families, and interdisciplinary staff
- Shift attention between multiple tasks and priorities in a dynamic setting
- Build trust with patients and families to foster positive experiences and engagement

- Shift: 7:00 AM – 3:00 PM

- Hours: 36 guaranteed hours per week
- Scheduling: Self-schedule model
