Shift: 7:00 AM – 3:00 PM
Contract Duration: 13 weeks
Schedule: Self-scheduled; floating as needed
Guaranteed Hours: 36 hours/week
About the Role
The facility is seeking a Case Manager to join a dynamic healthcare team. You'll work collaboratively with nursing, physicians, and interdisciplinary staff to coordinate patient care, manage documentation, and support positive patient outcomes.
Key Responsibilities
- Document patient information clearly and accurately in the facility's systems
- Communicate effectively with patients, families, and the care team
- Manage multiple priorities and shift attention between concurrent tasks
- Build trust with patients, families, and team members
- Support care coordination across the continuum
