Position Overview
The facility is seeking a Case Manager for a 13-week contract assignment. This is a day shift role (7:00 AM – 3:00 PM) with 36 guaranteed hours per week.
About the Role
You'll join a collaborative healthcare team in a fast-paced clinical environment. As a Case Manager, you'll work across multiple units, supporting patient care coordination and discharge planning. Your responsibilities will include:
- Collaborating with nursing, medical, and interdisciplinary teams
- Documenting thoroughly and managing competing priorities
- Building strong relationships with patients, families, and staff
- Coordinating care transitions and post-acute placement
