About the Role
The facility is seeking a compassionate RN to deliver skilled nursing care in a long-term care environment. You'll manage a diverse resident population with chronic conditions, post-acute rehabilitation needs, and complex medical requirements. This night-shift position offers the opportunity to make a meaningful impact on residents' quality of life and recovery.
Your Responsibilities
- Perform comprehensive assessments and monitor residents for changes in condition, escalating care as needed
- Administer medications and treatments per physician orders
- Manage chronic conditions, wound care, and rehabilitation needs
- Coordinate interdisciplinary care with physicians, therapists, dietitians, and nursing staff
- Document assessments, interventions, and resident response in the medical record, including MDS/PPS documentation
- Educate residents and families on care plans and condition management
- Support infection control, fall prevention, and skin integrity protocols
- Participate in care planning meetings and regulatory compliance activities
- Collaborate with CNAs and support staff to assist residents with activities of daily living
Required Qualifications
- Active New York State RN license (primary source verification required)
- Associate's Degree in Nursing (ADN/ASN); BSN preferred
- AHA BLS certification
- Minimum 1 year of skilled nursing or long-term care experience
- Experience with MDS assessments and geriatric care preferred
